Boston Sensory Solutions, LLC is an in-network health care provider for Blue Cross Blue Shield of MA & Harvard Pilgrim Healthcare.  Co-pays and/ or deductibles only will apply per visit.     We are an out-of-network provider for all other insurance companies, requiring payment by cash, check, or credit card at time of visit.  You can submit receipts for reimbursement to your insurer.

Prior to scheduling an appointment, please visit the "Documents" page.  Here you will see the forms that need to be completed prior to your first appointment.

1- For an adult or a child's appointment  Complete and fax or scan back to us the "Client Demographics Intake" page as soon as possible.  This will allow us to verify your insurance coverage and let you know if you need a primary care physician referral before scheduling your appointment.  Referrals should be made for "OT evaluation and treatment by Boston Sensory Solutions, LLC  NPI 1417308008."

2- Next, complete and fax or scan back to us the "Developmental History" form in full.  This lengthy form MUST BE competed and returned at least 7 days prior to your child's first appointment.  For adults, please contact us for relevant patient history questionnaires.

3- Finally, complete, and return on or before your first appointment, the "Consent Forms".  These may be returned prior to first appointment by fax or scanned into e-mail.  OR they may be brought to the first appointment.
